†Important note: If your Xiidra® iinsiderTM program card is inactive for 12 months, the card will be disabled. Please see your Eye Care Professional for a new card.

What do I do if I lose my card?

If you lose your card, please see your Eye Care Professional for a replacement card.

What does the number on the card mean?

The Membership Certificate number is your unique number that you should keep for record-keeping purposes.

Do you have to bring your card to the pharmacist each time?

The first time you wish to use your card, bring it to your pharmacist so they can have it registered. If you go to the same pharmacy at later visits, your card will be on file and you will not have to bring your card. If you go to a different pharmacy, you will have to bring your card.

Can I get multiple boxes (several months’ supply) at the pharmacy at one time?

Yes, you can get multiple boxes at one time provided your prescription allows for it. You cannot get more than 12 boxes over the course of one year (12 months).

Can I use the card if I have an insurance plan?

Yes, you can use your card if you have an insurance plan. Your card will pay any remaining fees above what your insurance plan covers.

Will the reimbursement be applied automatically?

Yes, the reimbursement will be applied automatically when your card is used. If you have insurance, your card will pay any remaining fees above what your insurance plan covers.

What happens if I don’t use my card for a long period of time?

Your card will be deactivated if you don’t use it for one year (12 months). If your card is deactivated, please see your Eye Care Professional for a replacement card.

Do I have to sign up for the program before using my card?

No, for the first 2 months, you do not have to sign up for the program before using your card. Your card is immediately ready to use. Simply take your card to your pharmacist and use it when filling your first prescription. However, registration is needed for continuous coverage.

Will the program have access to my personal information?

Yes, the program does need to collect basic information related to your personal identification. Your pharmacist may need to know several pieces of information to best serve you. Any information gathered by your pharmacist will be stored confidentially in your record at the pharmacy.

Do I have to get a new card every year?

No, you do not have to get a new card every year. You can continue to use the same card with future prescriptions.

Can I use the card after the first year?

Yes, your card can be used after the first year (12 months). Your card does not expire so you can continue to use it as long as you have a prescription. Please note that the Xiidra® iinsiderTM program offer can change without notice. You can visit www.xiidra.ca for the latest offer details.
